J.D. and Turk attend a medical conference in Phoenix, where they unexpectedly encounter J.D.'s former girlfriend, Dr. Kim Briggs, who has some shocking news for him. Back at Sacred Heart, the janitor takes on the role of chief of medicine, leading to comedic confusion. Simultaneously, Dr. Cox plants seeds of doubt in Elliot's mind regarding her upcoming marriage, causing her to question her future.
